In this vital role, you will work with a dynamic and engaging team of physicians, nurse practitioners, and other multi-disciplinary team members to provide primary health care to geriatric outpatients in the Hebrew Senior Life primary care clinic at Orchard Cove. Outpatient primary care will focus on disease prevention, wellness, and management of chronic and episodic illness. You will manage episodic and chronic medical needs of the residents. You will also participate in quality assessment activities including but not limited to record reviews, outcome measurement and review and revision of policies and procedures. It is essential for you to maintain accurate documentation of all services provided according to professional standards using electronic medical record. We will rely upon your knowledge to administer education and training to residents, caregivers and staff in relevant topics. You will effectively assess medical necessity and appropriateness of therapies and ancillary services. Masters Degree from an accredited Nurse Practitioner program is required. You must also be licensed by the Mass Board of Registration in nursing, and be nationally certified as a Geriatric, Adult, or Family Nurse Practitioner. You must have strong knowledge of the common geriatric disease stages, such as dementia, COPD, DM, and CHF. Expertise in the area of advance care planning and palliative care is a plus. It is essential for you to have strong verbal and written communicate skills in order to communicate effectively with patients, family members, colleagues, and other health care providers.
